Mounjaro: Medication for Diabetes and Obesity Treatment
Mounjaro is an innovative medication for treating obesity and/or diabetes, approved by both the FDA and the Israeli Ministry of Health for weight-loss treatment. Mounjaro operates via a combined, advanced mechanism: it slows gastric emptying, enhances feelings of fullness after meals, and helps regulate blood sugar levels.
- Age 18 and older.
- BMI of 30 or above, defined as severe obesity.
- BMI of 27 or above, defined as overweight, provided at least one additional obesity-related risk factor is present, such as diabetes, high blood pressure, or elevated blood lipid levels.
How does Tirzepatide work in the body?
Tirzepatide (Mounjaro) is an innovative medication, a synthetic derivative of the natural hormone GIP (glucose-dependent insulinotropic polypeptide). It belongs to a new class of drugs where the active ingredient interacts with GIP receptors in the body. This interaction enhances insulin secretion from pancreatic cells, helping transfer sugar from the bloodstream into cells and regulate elevated blood sugar levels after meals.
Recent studies have also highlighted a connection between GIP and the body’s fat metabolism processes. Additionally, the GIP hormone has been found to reduce food intake and simultaneously enhance calorie burning, highlighting its importance in obesity management.
Furthermore, GIP interacts with GLP-1 receptors, similar to medications such as Wegovy and Ozempic, thereby contributing to several mechanisms.
- Slowing gastric emptying: When digestion slows down, nutrients are released gradually, resulting in a prolonged feeling of fullness.
- Increasing insulin secretion from the pancreas: This action helps regulate blood sugar levels after meals.
- Reducing sugar release from the liver: The liver can release additional sugar into the bloodstream as needed. Tirzepatide prevents excessive sugar release from the liver into the bloodstream.
Tirzepatide’s uniqueness lies in combining two synergistic mechanisms: its effects on both GIP and GLP-1 receptors give it a broader therapeutic potential compared to medications affecting only a single receptor pathway, particularly for significant weight reduction and blood sugar regulation.
Additionally, pre-metabolic studies have highlighted another impressive benefit Tirzepatide helps maintain weight-loss results over time. This is due to its ability to moderate a bodily process called “metabolic adaptation,” whereby the body naturally strives to return to its original “set-point” weight after dieting. Tirzepatide reduces this metabolic resistance, helping prevent weight regain, a common issue faced after dieting efforts.

What studies have been conducted with Mounjaro?
The effectiveness of Tirzepatide (Mounjaro) as a treatment for diabetes and obesity among diabetic patients was evaluated in a series of Phase 3 clinical trials known as the SURPASS studies. These trials examined the medication’s efficacy both as a standalone treatment, as an adjunct to other medications, or compared directly with alternative therapies.
Here is a summary of the studies:
SURPASS 1 Study – examined the effectiveness of Tirzepatide as a standalone treatment at three different doses (5 mg, 10 mg, and 15 mg) among patients with type 2 diabetes who had elevated hemoglobin A1C despite following balanced nutrition and regular physical activity. After 40 weeks, results indicated a significant improvement in blood sugar regulation and body weight without increasing the risk of hypoglycemia (a sharp drop in blood sugar levels). Tirzepatide showed a dose-dependent weight loss ranging from 7 to 9.5 kg. Additionally, up to 92% of participants taking Tirzepatide achieved an A1C level below 7%, and up to 52% reached levels below 5.7%. Thus, Tirzepatide demonstrated considerable effectiveness in controlling blood sugar and reducing body weight.
SURPASS 2 Study – evaluated Tirzepatide’s efficacy at the same three dosages compared to a weekly injection of 1 mg semaglutide, the active ingredient in Ozempic (Wegovy also contains semaglutide, but at a higher dosage of 2.4 mg for weight loss). After 40 weeks, Tirzepatide was associated with significantly greater reductions in body weight and hemoglobin A1C levels compared to semaglutide (Ozempic 1 mg). Additionally, all three Tirzepatide doses positively influenced blood lipid levels, including triglycerides, bad cholesterol (VLDL), and good cholesterol (HDL).
- Participants taking Tirzepatide at a 5 mg dose lost an average of 7.8 kg.
- Participants taking Tirzepatide at a 10 mg dose lost an average of 10.3 kg.
- Participants taking Tirzepatide at a 15 mg dose lost an average of 12.5 kg.
In contrast, participants receiving Semaglutide (Ozempic 1 mg) lost only 6.2 kg on average.
Further analysis of the study showed that 60% of participants on Tirzepatide 15 mg and 51% on Tirzepatide 10 mg lost at least 10% of their initial body weight.

SURPASS 3 Study – This study evaluated the efficacy of Tirzepatide compared to insulin degludec among diabetic patients with poorly controlled blood sugar levels on metformin (Glucophage). Results indicated that Tirzepatide at the highest dosage (15 mg) led to a significantly greater reduction in hemoglobin A1C compared to insulin degludec. Participants taking Tirzepatide lost an average of 7.5 kg (5 mg dose), 10.5 kg (10 mg dose), and 12.9 kg (15 mg dose), whereas the insulin degludec group experienced an average weight gain of 2.3 kg.
SURPASS 4 Study – In this trial, involving participants at increased cardiovascular risk, all three Tirzepatide dosages achieved greater reductions in hemoglobin A1C compared to insulin glargine. Additionally, treatment with Tirzepatide resulted in significantly greater weight loss.
SURPASS 5 Study – Participants in this trial were type 2 diabetes patients treated with insulin glargine, with or without metformin. After 40 weeks, adding Tirzepatide at a 15 mg dose to insulin glargine significantly lowered hemoglobin A1C by 2.59% compared to only 0.93% in the placebo group. Tirzepatide also led to an average weight loss of 10.9 kg and allowed for a reduction in insulin dosage, compared to the placebo group, which saw an average weight gain of 1.7 kg and a 75% increase in insulin dosage.

Frequently Asked Questions (FAQ)
At the start of treatment, your doctor will determine the weekly dosage of Mounjaro. Typically, the dosage is gradually increased based on your rate of weight loss until reaching the optimal dosage, at which you'll remain until the completion of treatment.
Tirzepatide (Mounjaro) can be injected at any time of day, but it's advisable to inject it at the same time each day.
The overall safety profile of Tirzepatide (Mounjaro), as shown by studies thus far, is similar to that of other GLP-1 medications. Common side effects include nausea, diarrhea, and vomiting. Comparable side effects have also been observed with other medications, as evidenced in the SURPASS 2 study, where Ozempic users experienced similar side effects to those taking Tirzepatide.
It's important to remember that your physician, especially one who specializes in obesity treatment, can advise you on how to significantly reduce or even prevent medication side effects. Real-time dynamic adjustments of the treatment plan, including dosage changes and monitoring interactions with other medications, can greatly improve patient adherence and reduce treatment dropout rates.
Rare side effects may include hypoglycemia, severe allergic reactions, or jaundice.
Tirzepatide is not recommended for individuals with severe heart failure or serious gastrointestinal conditions causing delayed gastric emptying (gastroparesis) or inflammatory bowel diseases. Additionally, you should inform your doctor if you have any kidney or pancreas-related medical conditions.
The medication is not included in Israel's national health basket.
Currently, some health funds (Kupot Holim) offer up to a 50% discount on Mounjaro, subject to each health fund's specific criteria.
The consumer price ranges from approximately 700 NIS for lower dosages up to 1,800 NIS for higher dosages.
