How does Saxenda work in the body?
Saxenda is a medication based on a molecule similar in structure and function to a natural human hormone called GLP-1, which is released whenever we eat. This hormone decreases appetite and enhances feelings of fullness after meals. The active ingredient in Saxenda mimics the action of GLP-1 by interacting with nerve cells (neurons) in the brain responsible for regulating appetite, while also slowing down the emptying of food from the stomach. Thus, Saxenda belongs to a class of medications known as “Incretin Mimetics.”
In fact, Saxenda contains the same active substance as the medication Victoza, used in diabetes treatment. Victoza helps regulate blood sugar by increasing insulin secretion from pancreatic beta cells in response to eating food. The key difference between these two medications lies in their prescribed dosages. It was discovered that starting from a certain dosage, this active ingredient not only helps manage blood sugar levels but also contributes to weight loss.
What is the research basis for the effectiveness of Saxenda?
Saxenda’s efficacy and safety for the treatment of overweight and obesity have been proven through four placebo-controlled clinical studies, involving a total of over 5,000 participants. The largest study examining the effectiveness and safety of Saxenda included 3,731 participants and lasted for 56 weeks \[1].
At the end of this study period, Saxenda contributed to an average body weight reduction of 8%, compared to just 2.6% in participants who took a placebo and relied solely on dietary changes and physical activity. Additionally, 63.5% of participants treated with Saxenda lost 5% or more of their body weight, compared to only 26.6% in the placebo group.
Furthermore, 33% of Saxenda users achieved a weight loss of over 10%, compared to just 10% of those receiving the placebo.

In another placebo-controlled clinical trial \[2], involving 846 participants with overweight or obesity and type 2 diabetes, Saxenda demonstrated effectiveness and safety in promoting weight loss when combined with diet and exercise. After 56 weeks, 54.3% of participants taking Saxenda lost 5% or more of their body weight, compared to only 21.4% in the placebo group. Furthermore, 25.2% of Saxenda users achieved a weight reduction of more than 10%, while only 6.7% of placebo users reached this threshold.
What about diabetes management? Saxenda was found not only effective for weight loss but also in reducing the risk of developing diabetes. This was demonstrated in a 56-week clinical study involving participants with pre-diabetes and a BMI of 30 or higher, or a BMI of 27 or higher along with weight-related health conditions (such as hypertension) \[3].
Studies have also shown that the therapeutic effectiveness of Saxenda lasts for at least three years, helping patients maintain their weight loss results over time. As a result, the FDA has approved Saxenda for long-term treatment.

Rare side effects may include severe allergic reactions (anaphylaxis), reduced kidney function or acute kidney failure, and liver or pancreas damage. (It's important to note that obesity itself triples the risk of pancreatitis.)
Saxenda is not recommended for individuals suffering from severe heart failure, serious gastrointestinal conditions that cause delayed stomach emptying (gastroparesis), or inflammatory bowel diseases. Additionally, the medication may increase the risk of thyroid issues, including thyroid malignancy. Caution is also advised in situations that increase the risk of pancreatitis.
Patients taking Coumadin may require regular monitoring of blood clotting parameters.
Patients using glimepiride or glibenclamide may need dosage adjustments by their doctor to prevent excessively low blood sugar levels.
